QUALIFYING CONDITIONS FOR MEDICAL CANNABIS

Patients will first need an official medical diagnosis. Under Amendment 2, a wide range of conditions may be eligible for medical cannabis. The following are qualifying conditions:

  • Cancer 

  • Epilepsy 

  • Glaucoma 

  • HIV

  • AIDS

  • Post-traumtic stress disorder (PTSD) 

  • Amyotrophic lateral sclerosis (ALS) 

  • Crohn's disease 

  • Parkinson's disease 

  • Multiple Sclerosis 

  • Terminal Illness

  • Chronic pain

Other debilitating medical conditions including but not limited to: 

  • Anxiety 

  • Depression

  • Endometriosis 

  • IBS

  • Gastrointestinal disorders

  • Autoimmune disease 

  • Sleep disorders 

  • Arthritis 

  • Fibromyalgia

  • Neuropathy 

"Other debilitating medical conditions" are medical conditions that include symptoms similar to those experienced in the specific qualifying conditions. If a patient is suffering and you think medical cannabis may help, please contact us to discuss further.

STEP 1: INTAKE APPOINTMENT
Intake Appointment

Request your initial appointment today. Just fill out the contact form or call us at (850) 863-3000 to request an appointment and we will get back to you as soon as possible to schedule an appointment.

It is highly advisable to bring your medical records documenting your qualifying condition to your appointment, this will expedite this process.  Medical records from your doctor will provide us with the details of your illness which allows us to qualify you for a medical cannabis ID card. At your initial evaluation the doctor will review your health conditions, enter you into the Medical Marijuana Use Registry and discuss your individualized treatment plan. 

STEP 2: REGISTER FOR YOUR ID CARD

We will enter you into the Medical Marijuana Use Registry during your initial appointment. The Office of Medical Marijuana Use (OMMU) will then email you a login. In order to receive the required medical cannabis ID card, you must submit a completed application to the OMMU. This includes: an approved passport-type photo, a copy of the required proof of residency, $75.00 check or money order made out to the Florida Department of Health. We will be happy to help you through this process.

STEP 3: OBTAIN YOUR MEDICINE

Once you receive your temporary or permanent medical cannabis ID card you may purchase medication at a state licensed medical cannabis dispensary. We file prescriptions electronically and work with dispensaries directly on our patients behalf. We focus on customizing treatment plans with attention to detail including optimizing dosing and delivery regimens.
